Social Worker, Permanent Housing in New York

The Social Worker under the direction of the Program Coordinator is responsible for initial and ongoing needs assessment,

individual service plans, referral and advocacy for individuals who reside in permanent supportive housing for women and

women with children who have experienced chronic homelessness, mental illness or substance use disorder or a physical

disability. The Social Worker’s caseload may include both single women and women with children. The position will require

occasional weekend and evening work hours. The Social Worker will offer specialized services focused on addressing the

complex issues of trauma, mental illness, addiction, family stability, positive parenting and family interaction designed to

support housing stability, family stabilization, independent living, financial security.
