The Master Social Worker provides counseling, psychosocial evaluation, and resource referral services to patients at our facilities, and remotely. They collaborate with the treatment team, advocate for patient needs, and maintain accurate documentation.

Responsibilities:

• Conduct comprehensive psychosocial assessments and provide counseling services to patients and their families

• Offer case management and coordinate services to optimize patient functioning

• Refer patients to community services and work with the interdisciplinary team for care planning

• Develop resources to meet patient needs and participate in orientation, education, and rehabilitation programs
