Home Health Occupational Therapist - $5,000 Sign on Bonus in New Mexico

We are currently seeking a qualified full-time Occupational Therapist to provide care for our patients in Valencia County. Under the supervision of the Home Health Director and in accordance with the interdisciplinary plan of care, our therapists evaluate, establish and implement individual programs of care for our patients to increase functional ability and reestablish independence at home.

Responsibilities & Duties:

• Performing home visits to evaluate patient’s level of function by utilizing diagnostic and prognostic procedures
• Developing individual plan of care based on patient's personal goals
• Supervising and directing Certified Occupational Therapy Assistants with regard to patient care
