Home Health Admissions Physical Therapist (PT) in New Mexico

As a full-service hospice and home care agency providing a broad range of services to our customers, we are currently seeking a Physical Therapist to establish care for our patients in their home environment. Under the Home Health Director's supervision, our therapists perform patient assessments and implement individualized plans of care to help our clients achieve their functional goals.

Responsibilities & Duties:

• Performing home visits to evaluate patient's level of function
• Developing an individual plan of care based on the client's personal goals
