The Hospice CDM nurse helps bridge the gaps between Home Health and Hospice patients by identifying the needs, recommending resources, communicating with patient's healthcare providers and educating on disease progression and treatment/care options available. Often times people make critical healthcare decisions in crisis and Phoenix's CDM nurses are able to walk this journey with people so they can make informed decisions that align with their personal goals.
