Family Nurse Practitioner in Minnesota

Schedule: 30 or 32 hours per week, flexible days 8am-4pm or 9am-5pm.

The Family Nurse Practitioner will provide professional, quality services to the clients served by RADIAS Health Homeless Services. Diagnose, prescribe medication, monitor medication side effects, and participate in the treatment process of clients served by the Homeless Services Team.

Duties & Responsibilities:

• Provide street medicine directly to individuals experiencing unsheltered homelessness in Ramsey County, on their own terms and in their own lived environment, to build relationships and deliver medically necessary services in order for individuals to stay healthier to avoid unnecessary Emergency Department visits and hospitalizations.
