Behavioral Health Case Manager in Massachusetts

Mental Health Association is hiring immediately for a Behavioral Health Case Manager, fluent in Spanish. The Behavioral Health Case Manager will provide outreach services, case management, care coordination, and facilitate community linkages to participants of the BestLife Emotional Health & Wellness Center. These efforts are Person-Centered and in the interest of maintaining optimal functioning in the community. Perform responsibilities professionally, in accordance with MHA’s Core Values (Respect, Integrity, and Compassion), policies, practices, program funding and applicable regulatory agency guidelines.
