Eminence Home Care is seeking a dedicated and experienced Home Care Assessor to assist with the screening and onboarding of new clients. The ideal candidate will have a background in care management and possess strong intake and assessment skills. This role involves evaluating clients' functional needs, home environments, and safety, as well as verifying basic funding sources for care.
