Medical Director of Population Health in California

This clinical position oversees Tiburcio Vasquez’s Health Centers (TVHC) Population Health Department, whose mission is to improve the health of our patients and our community by addressing disparities through health education, patient empowerment, and advocacy. Directly reporting to the CMO, and as a member of the management team, TVHC’s Medical Director of Population Health (MDPH) provides leadership, motivation and direction across TVHC to ensure that the organization supports best population health practices. The MDPH provides guidance to Program Directors & Managers of our Population Health programs.
